Telemedicine and Parkinson disease: Improving Care and Clinical Research


Google Tech Talk March 22, 2010 ABSTRACT Presented by Kevin Biglan, MD, MPH and Ray Dorsey, MD, MBA, University of Rochester. The cost of clinical trials is rising, recruiting participants is increasingly difficulty, and monitoring safety is increasingly important. Home and patient appropriate technologies like the Marvell Plug computer and Google products can help address all of these issues. We will discuss our efforts to monitor blood pressure remotely using these technologies in a Michael J. Fox funded Parkinson disease clinical trial. Telemedicine using simple web-based video conferencing using, for example, the Marvell Plug computer and Google's services can also address large unmet needs in clinical care. Chronic conditions account for 75% of health care expenditures and affect over 140 million Americans. However, many have limited access to physicians, especially specialists, who can improve their care. We will present results of a pilot, randomized controlled trial of increasing specialty access using telemedicine to individuals with Parkinson disease residing in a remote nursing home and remote communities. We discuss future directions, including providing care directly into people's homes ('virtual house calls'), and ways that Google's services can provide additional value. A new variation of the Marvell Plug computer in planned as an interactive television server for the virtual house calls in a upcoming study.

In this video—presented by the National Heart, Lung, and Blood Institute, part of the National Institutes of Health—NHLBI grantee Dr. Edwin Silverman of Brigham and Women's Hospital in Boston talks about COPD Gene, which he runs with Dr. James Crapo of National Jewish Health in Denver, and about the importance of participation in clinical studies. COPD Gene is a study of more than 10000 volunteers across the country who are undergoing genetic assessment, state-of-the-art CAT scan imaging, breathing tests and other measurements in order to better understand why some people get COPD (chronic obstructive pulmonary disease) and others don't. The goal is to identify the genes that play a role in the development of COPD and to ultimately improve the prediction, diagnosis, and treatment of the disease. "This research would not have been possible without the incredible support from the general population," said Dr. Silverman. "If we hadn't had enthusiastic support from our volunteers who gave up their own time to come in, get a CAT scan, go through an extensive array of tests with breathing tests and walk tests and a long list of questionnaires, if they hadn't been willing to do that, we just couldn't do the work. GEICO Employees Improve Health Dramatically with Low-Fat Vegetarian Diet


Hillary and Bruce, who followed a low-fat vegetarian diet in a GEICO worksite study, exemplify how office workers can lose weight, lower blood pressure, and reduce absenteeism if their employer provides healthy low-fat vegetarian meals in the company cafeteria. The findings are published in a new study published in July's American Journal of Health Promotion. "GEICO's workplace nutrition program helped employees lose weight and reduce blood pressure and cholesterol levels," says Susan Levin, MS, RD, an author of the new study. "If a company cafeteria offers low-fat vegetarian options every day, employees' heath improves and they miss less work." Ms. Levin is director of nutrition education for the nonprofit Washington Center for Clinical Research.

Richard Doll was a luminary of clinical research whose case control study, published in the BMJ in 1950, first identified smoking as an important cause of cancer and other diseases. He carried his research out on doctors in the UK who smoked, and tracked their mortality over the course of 50 years. The latest paper being published in the BMJ in 2004.
